Application for student assistants and list of requirements

REQUIREMENTS FOR STUDENT ASSISTANCE PROGRAM:

1. Form 138 or high school card
2. College Scholastic Aptitude Test (CSAT)
3. Birth Certificate
4. Baptismal Certificate
5. One copy of 2x2 colored picture
6. Certififcate of Tax Exemption from BIR (of parents or guardian)
7. Good moral character recommendation from the cognizant principal or parish priest
8. In case of students who are currently enrolled in Columban College or in other educational institutions (tertiary level) who would like to participate in this program, applicants must not have completed 70 units. They must also submit an original copy of their transcript of records and above requirements (1) thru (6), as appropriate.
